Install

dsh plugin --profile web add dsh-plugin-agent-monitor

Or install it as a local package: symlink the directory into the dsh profile's node_modules, add "dsh-plugin-agent-monitor" to dsh.profile.bundles in ~/.dsh/profiles/web/package.json, and restart dsh web.
